Regularly a member of the Japanese national team, she was awarded an Olympic silver medal in the 2012 Summer Olympics and bronze medal at the in 2016 Summer Olympics.
Within Japan, Ishikawa has ranked among the top five female athletes every year since 2014 until 2019 in annual lists released by Oricon. Because she speaks fluently Mandarin which she was taught by Chinese coach, her fame has also spread to China. When she launched an account on Sina Weibo account in 2016 the account gained more than 60k followers in the very first day.
Ishikawa appeared in the 2017 film Mixed Doubles as herself.
At the 2017 World Table Tennis Championships held in Düsseldorf, partnering compatriot Maharu Yoshimura in the mixed doubles event, Ishikawa and Yoshimura progressed through to the final, beating the combination of Fang Bo and Petrissa Solja in the semi-finals, and overcame the Taiwanese pair of Chen Chien-an and Cheng I-ching 4–3 to win the first title in this tournament. It was also Japan’s first mixed doubles gold medal after 48 years.
She reached the semi-finals in the Women’s Singles competition at the London 2012 Olympics but was eliminated by the eventual gold medalist; Li Xiaoxia of China. In the play-off for bronze medals she was eliminated by Feng Tianwei of Singapore. She was placed in the overall 4th place. This was her first Olympics participation, and despite the relatively young age of just 19, she was seeded No. 4 in singles for women in the Event. It is also the first time ever Japanese to reach the semi-finals in the tournament.

After graduating of Hirakawa Elementary School, located situated in Yamaguchi, Ishikawa wrote in the yearbook her desire to compete for Japan in the Olympics. Her first public recognition came when she was able to beat high school and students from universities that were older than her at her first All Japan Table Tennis Championships. Since 2007 she has been the winner of all four of the All Japan Table Tennis Championships in the female junior division four times in succession. Her nickname was Ai-Chan II to refer to her fellow player Ai Fukuhara who began playing table tennis at an early age. She modelled her underspin serves on Fukuhara.
She was a participant in the 2009 World Table Tennis Championships, reaching the quarterfinals of the singles event. She was awarded the title of the nation’s singles champion at 17 years old. She swiftly rose through the rankings of the world in recent years, and was able to secure the right to play at her first London 2012 Olympics. She currently stands at sixth in the ITTF women’s world rankings in July 2012, but she has reached up to 5th. She is now the most powerful women table tennis athlete in Japan after she has managed to beat Ai Fukuhara in rankings.
She won bronze with the Japanese Olympic team in Rio de Janeiro 2016, beating together with Ai Fukuhara and Mima Ito on her side the team from Singapore. She also represented Japan in the women’s singles event, where she crashed out of the tournament after suffering a cramp in her leg, losing 4–3 to North Korea’s Kim Song-i in their third-round match.
