Despite failing to be a part of it into the Cork Under-21 squad, McCarthy was drafted onto the Cork senior squad in November 2008, as part of the “development squad” that replaced those who were part of the regular team who were out on strike. McCarthy made his first appearance with the team on the 11th of January, 2009 in a 0-14-to 0,09 defeat against Cork’s Waterford Institute of Technology in the Waterford Crystal Cup. After losing the 2009 final to Newrtownshandrum, Sarsfields were back in a third successive decider on 10 October 2010. McCarthy top scored for the team with eight points in the 1-17 to 0-18 defeat of Glen Rovers.
McCarthy made his debut as a member of his Sarsfields Senior team the 3rd of May, 2008, when they defeated the Sarsfields senior team in a 2-9 to 2 defeat to Ballinhassig. On the 28th of September, the 28th of September, McCarthy was a right wing-forward for his first final of the senior championship. McCarthy scored a point during play in a 2-14 – 2-13 win over Bride Rovers to claim his first Cork Senior Championship medal.
