Underwood was selected to the Detroit Tigers in the first round of the 1976 Major League Baseball Draft, as the second overall pick.
He was chosen from the Texas Rangers on December 5 of 1983 after being selected by the Reds in the 1983 Rule 5 draft, but the Rangers let him go before the beginning in the season of 1984. He was signed by the Baltimore Orioles on April 2 1984 as a no-cost agent. He was a player in 1984 with the Rochester Red Wings, the Orioles A-A affiliate and posted a 0-2 record as well as an ERA of 9.00 in three relief appearances.
He spent the whole 1981 season playing for the Tigers and was able to switch between starting and relieving. He posted a 3-6 record during the season, and five saves with an ERA of 3.59. The Tigers also faced The Chicago White Sox on the famous Disco Demolition Night at Comiskey Park in which he pitched 7 1/3 innings while allowing just one hit in what was supposed to be game one of the doubleheader. As a member of the Tigers the team, he posted a record of 6-4 and as a result, an ERA that was 4.59 which included 83 hits across 27 games.
With the Tigers in 1983, he pitched in four games and had an ERA of 8.71. His final major league appearance was on June 21, 1983, when he gave up five runs in two innings as part of a 10-3 loss to the Milwaukee Brewers. The Tigers traded him on June 30, 1983 to the Cincinnati Reds in exchange for third baseman Wayne Krenchicki. He split the season in the minor leagues between Evansville with the Tigers and at the Indianapolis Indians of the International League for the Reds. Between the two teams he had a record of 7-4 with a 4.09 ERA.
