Fahey’s 1997-98 season marked the beginning of a new era. The Bears won the national championship 77-69 over Southern Maine with a 28-2 record. They went undefeated with 30-0 and won the national championship against Saint Benedict 74-65. The following year was similar, finishing 30-0 and winning the national championship over Saint Benedict 74-65. The team was defeated in 2000-01 but they finished 28-2 and won their fourth consecutive national championship against Messiah College 67-45.

Over the next seven seasons, Fahey would lead Washington University to another four conference championships and advance to the quarterfinals twice (in 2015-16 and 2016–17).

On March 22, 2017, Fahey was named as head women’s basketball coach at the University of Illinois, following former Washington University athletic director Josh Whitman to the school after he left for Illinois the previous year.

From 1997-98 through 2006-07 Washington won the UAA conference. In 2007, Washington reached the national championship. They lost 55-52 to DePauw. 2007-08 was the first season in which the Bears had failed to win 20 games. They went 19-8 and finished second in the conference. They rebounded in the following year, finishing 26-5, losing the national championship match to George Fox University 60-53. The Bears won their fifth national championship in 2009-10. They finished the season at 29-2, defeating Hope College 65 to 59, and took home the title. The Bears finished second in the UAA the following year and went on to the national championship game, losing to Amherst 64 to 55.

Fahey started her coaching career right after she graduated from Wisconsin. In 1982, Fahey was appointed Johnsburg High School’s girls basketball coach. In her final two years, she led the Lady Skyhawks basketball team to 20 wins and regional championships.

Fahey was appointed head women’s basketball coach at Washington University in 1986. Fahey was successful in her first season with a record 16-5. The school joined the newly formed University Athletic Association, and competition began in 1987-1988. Washington won the first three of the eight UAA championships and seven of the eight first eight. This made the NCAA Division III Women’s Basketball Tournament seven out of eight years. Fahey finished fourth in 1991’s NCAA Tournament, despite finishing second at the UAA. She lost in the Final Four to St. Thomas in 1991 and was defeated by Eastern Connecticut in the third-place match. The Bears were back in the tournament in 1994. They reached the championship round before falling to Capital University.

After winning her fifth national championship, Fahey became the first NCAA Division III coach to be admitted to the Women’s Basketball Hall of Fame in the Class of 2012.
