He was diagnosed with a malignant brain tumour in autumn 2000. After radiation therapy, he was able to resume Bundesliga action in 2001. However, he was unable to return to his former glory days as a player. After several injuries, he ended his professional career.

Herrlich was a German Bundesliga player who played 258 matches (1989-194) and scored 75 goals for Borussia Dortmund, Borussia Monchengladbach, and Bayer 04 Leverkusen. He won the German championship with Borussia Dortmund in 1996 and 2002, as well as the Champions League in 1997 and Intercontinental Cup 1997. He won the 1995 DFB-Pokal with Borussia Monchengladbach, which he also won with Bayer Leverkusen in 1993.
He played five matches in 1995 for the German national soccer team. They were all qualifying matches for UEFA Euro 1996. He scored against Wales once but was unable to play in the final tournament which Germany won due to injury.
In July 2007, he was appointed as coach of the Germany U-17 team and won third place at the 2007 FIFA Under-17 World Cup in South Korea. On 27 October 2009, he was released from the German Football Association. He became later the head coach of VfL Bochum between 27 October 2009 and 29 April 2010. He finished with a record of four wins, eight draws, and 10 losses.
