Milone began his regular campaign in the 3rd starting position, which was occupied by Brandon McCarthy and Bartolo Colon. Milone had the distinction of being the one pitcher in the A’s lineup to remain for the entire season without being injured. He also played the most games with the A’s throughout the season of 2012. Milone pitched his debut complete game of his career on June 20, beating his team, the Los Angeles Dodgers. Milone was in the second game of the ALDS however, the A’s lost to a no-decision in the ninth inning’s bottom. Milone ended this season with record of 13-10 with 137 strikeouts, and an ERA of 3.74
Milone was optioned to the Triple-A Sacramento River Cats on August 3, 2013. Milone finished the season with 12 wins in 28 games, 26 of them starts.
On July 31, 2014, the Athletics traded Milone to the Minnesota Twins in exchange for outfielder Sam Fuld. Milone started in five games for the Twins before being shut down with a neck injury.
Milone made his debut in the major leagues in September 3rd, 2011 in a game against the New York Mets. Milone hit Angel Pagan of the New York Mets to record his first strikeout in his career and then hit a home run of three runs on the very first pitch in his debut Major League at bat, becoming the 27th player in Major League history to hit a home run. He was dismissed from the game after throwing four and one-third innings.

On December 20, 2017, the Washington Nationals signed Milone to a minor league contract, with an invite to spring training. On July 26, 2018, he was called up in place of Stephen Strasburg in the rotation. Milone was reassigned to the bullpen on August 18. On September 4, Milone was outrighted off the roster.
