After having graduated of Gauley Bridge High School in 1974, he enrolled in at the Columbus College of Art and Design as well as West Virginia University. From 1979 to 1981 , he was a student at The Kubert School in New Jersey.

A longtime music lover and fan, Truman has integrated his musical passion into illustration and comics. While working with Eclipse Comics, Truman included an Flexi disc recording in Scout Issue 19 (May 1987) that served as music for some of the stories of the comic. The album was also released on Eclipse Records with his band The Dixie Pistols entitled Marauder. The album also included an illustrated comic book that portrayed his wedding to Emanuel Santanna that took place between the beginning of the Scout series Scout and the sequel Scout: War Shaman. With Flint Henry, Truman co-authored Rifts Dimension Book 1: Wormwood (1993), which was critically acclaimed for its vibrant backgrounds.

He currently lives in Lancaster, Pennsylvania, and was an instructor at the Pennsylvania College of Art and Design from 2003 to 2006.

Truman’s first comics job as a professional was a collection of back-up stories published in DC Comics’ Sgt. Rock comic, which ran between 1981 and 1983. His first major comic was Grimjack along with the writer John Ostrander, for the independent comics publisher First Comics. Grimjack first was featured in Starslayer issue #10 (November 1983) and then moved to his own comic after issue 18 (July 1984) and continued for an additional 81 issues.

At DC Comics he created Hawkworld, a reinvention of Hawkman in 1989. An ongoing series with the same name was launched the following year. With author Joe R. Lansdale, he reinterpreted Jonah Hex as a horror western. In it, their creation of villain Edgar Autumn elicited a lawsuit from musician Edgar Winter.
