Ramos was recommended by Democrats of the Texas House delegation to fill the Corpus Christi vacancy at the United States District Court of the Southern District of Texas during the 111th Congress. John Cornyn and Kay Bailey Hutchison, Republican senators, also supported Ramos. Ramos was nominated by President Barack Obama to replace Judge Hayden Head on January 26, 2011. Ramos was confirmed by the United States Senate unanimously on August 2, 2011. On August 4, 2011, she was awarded her commission.

On April 10, 2017, Judge Ramos ruled that Texas’ voter ID law was passed in 2011 with the intent to discriminate against minority voters. On April 27, 2018, the Fifth Circuit Court of Appeals reversed Ramos’ ruling, upholding the Texas voter ID law in a 2–1 vote.
