After two assistant referees failed FIFA standards, he was promoted to officiate the matches at the FIFA World Cup finals 2006. After being promoted to officiate matches at the 2006 FIFA World Cup, he was assigned to three matches: Germany v. Poland and Argentina v. Argentina. He made controversial decisions in the round 16 match between Australia and Italy. He gave Marco Materazzi a straight yellow card for a challenge against Mark Bresciano of Australia. Later, he awarded Italy an injury penalty after judging Lucas Neill, Australian defender, to have fouled Fabio Grosso, full back to Italy. Francesco Totti scored the resulting spot kick and secured a 1-0 win for the Italians.

On 20 May 2009, Medina refereed the UEFA Cup Final between Shakhtar Donetsk and Werder Bremen.

Two days after the Australia–Italy game FIFA announced that Medina was one of the twelve referees retained for the remainder of the tournament. He refereed the quarter-final between Brazil and France on 1 July.

Cantalejo referred to the Russian Premier League match between Zenit St.Petersburg and CSKA Moscow on 21 August 2005.
