Otto was crowned Miss Pennsylvania two years later after she won the Miss Three Rivers title. She was awarded the Talent award in the preliminary competition, and she was crowned Miss Pennsylvania after the final competition ended on July 12, 2003. She was the first Miss Pennsylvania to be from Murrysville. She was awarded a $6,000 scholarship for her win.

She made 150 appearances as a keynote speaker and featured performer during her reign. Each month she drove between 2,000 to 4,000 miles across the state in a car, crisscrossing it by car. Her platform issue, “Challenging Americans To Address Poverty”, was inspired by her involvement with National Student Partnerships (NSP) in 2001. She was also a co-founder and a member of the advisory board for NSP Pittsburgh.

Otto competed in her first pageant in 1996, when she was Miss Pennsylvania Teen USA. While attending college in Evanston, Otto entered Miss Illinois 2001 and won the talent award for non-finalist.

Otto is a talented performer and singer. At six years old, she began acting classes. She continued to perform in minor roles in local newspapers. After taking voice lessons, she joined the chorus of Civic Light Opera. She graduated from Shadyside Academy in Pittsburgh, Pennsylvania in 1998. In 1998, she attended Northwestern University to study Musical Theater. Candace received the opportunity to study opera in Italy with the Ezio Pinza Council of American Singers of Opera after graduation. She performed in November 2001 with the Sistine Choir before a papal audience 250,000 at the Vatican City.

In September she went on to compete in the Miss America 2004 pageant where she performed “Non Ti Scordar Di Me” as her talent selection, the same song she performed in the state pageant. At the Miss America Pageant, she was the first Miss Pennsylvania to win the Bernie Wayne Performing Arts Award and the first Miss Pennsylvania since 1991 to win a non-finalist talent award.
